To view an example of each question type before you add it to your survey, hover over any question type in the BUILDER section of the left sidebar in the Design Survey section. To disable these examples, hover over any question type and select Hide Builder tooltips.

You can edit questions at any time, but some editing limitations apply once the survey has already collected responses.

Question Type | Description |
---|---|
Multiple Choice | Ask people to choose one answer from your list of answer choices. |
Checkboxes | Let people select multiple answers from your list of answer choices. |
Image Choice | Ask people to choose one or more image answers from your list of image answer choices. |
Dropdown | Provide a dropdown list of answer choices and ask people to choose one answer. This is a great way to save space if you have a long list of options, like country, state, or province. |
Star Rating | Ask respondents to evaluate a statement on a visual scale of stars, hearts, thumbs, or smilies. |
Matrix/Rating Scale | Use a Matrix/Rating Scale question if you want respondents to evaluate one or more row items using the same set of column choices. You can set the matrix question to collect multiple answers or only one answer per row. You also have the option to turn the matrix question into a rating scale when you want to assign weights to each answer choice. |
File Upload | Allow respondents to attach a file to their response. |
Slider | Ask respondents to rate an item or statement on a numerical scale by dragging an interactive slider. |
Matrix of Dropdown Menus | Ask respondents to evaluate several items using the same set of measurements. They can choose an answer from a preset list of answer choices presented in a dropdown menu. |
Ranking | Ask respondents to rank a list of options in the order they prefer using numeric dropdown menus or drag-and-drop. |
Net Promoter速 Score | Ask your customers the Net Promoter速 Score question to measure customer satisfaction with your organization, brand, product or service. We'll calculate your score automatically when you analyze your results. |
Single Textbox | Add a single textbox to your survey when you want respondents to write in a short text or numerical answer to your question. You can also specify answer length, and require and validate a number value, date format, or email address. |
Multiple Textboxes | Collect multiple short text or numerical answers with a single question. You can also specify answer length, and require and validate a number value, date format, or email address. |
Comment Box | Use the comment or essay box to collect open-ended, written feedback from respondents. |
Contact Information | Use this question type to collect a respondent's relevant contact information, including name, address, email, and phone number. |
Date/Time | Ask respondents to enter a specific date and/or time. |
Text | Insert additional text to your survey design to provide extra context or description to your respondents. |
Image | Insert an image into your survey by uploading an image file or specifying the URL of an image on the web. |
Text A/B Test | Learn how respondents react to two or more different text passages. Enter the text variables you want to compare, then specify the percentage of respondents who see each message. |
Image A/B Test | Learn how respondents react to two or more different images. Provide the images to compare, then specify the percentage of respondents who see each one. |

SurveyMonkey makes it easy to create a survey from scratch or from a template. Follow the steps below to get started.
To create and send a survey:
1. Create a Survey |
To create a new survey, click Create Survey in the upper-right corner of your account and choose one of the following options: Start from scratchGet started with a blank page. You'll need to add questions by using the tools in the left sidebar to build your survey. If you have questions ready to go and want to copy and paste them to your survey, select My questions are already written. Copy a past surveyYou can create a copy of a survey that's in your account. This way you don't have to start from scratch or change past surveys that have already collected responses. The survey questions, logic, theme, and other survey design settings are carried over to the copy, but you'll need to create a new collector to send your survey. Start from a templateGet a head start by using a template designed by our team of survey experts. Use the dropdown menu to browse templates by type to find the right survey for you. If you're on a team, you can also browse your team survey templates. Click on any template to learn more about it and preview the survey questions. Build it for meENGLISH ONLY: Build it for me is only available in English language accounts. Build it for me helps you create a survey designed to meet your goals. To get started, answer a few simple questions about you and your survey goals. Then, Genius Assistant gives you recommendations to finish personalizing your survey questions and design. When you’re ready to send, we’ll also recommend the best collector types and options for your survey. |
2. Add Questions & Pages |
Once you create your survey, use the left sidebar to add your own questions or choose pre-written questions from our Question Bank. You can apply question settings to further customize them. If you have a long survey, break it up into multiple pages. Pages help you organize your survey into sections, which makes it easier for people to take. Plus, every time they go to the next page, their previous responses are saved. If you're using a template, review the survey to customize questions and answer choices so it's relevant to your survey goals. For example, you can add the specific product or company name you're looking for feedback on. You may also want to add new questions or delete questions that aren't useful to you. |
3. Apply Logic |
After your survey questions and pages are set up, it's a good time to apply logic to the survey. Logic can help you get better data by creating a more personalized survey experience for survey takers. For example, you can use question skip logic to control which questions survey takers will see next based on their answers to previous questions. Or, you can use question randomization to reduce potential bias in the order questions are presented. |
4. Customize the Design |
After your questions and logic are in place, you can focus on fine-tuning the look and feel of your survey with our design and display options. For example, show or hide elements on the survey page, add a logo, or create a custom theme. |
5. Preview & Send Your Survey |
Always preview your survey before sending it to survey takers so you can catch any mistakes or logic issues. Once you're ready to send it out, create a collector to get responses. |
